Acne Marks vs Acne Scars: Know What You Are Treating

This distinction saves people months of frustration, because the two problems look related and respond to completely different things.

Marks are colour. Scars are texture.

Run a finger over the area, or look at it in side-lighting rather than straight on.

  • Flat and smooth, but discoloured — that is a mark. Brown, or pink to red.
  • Indented, raised, or uneven to the touch — that is a scar. The skin structure itself changed.

The two kinds of mark

Post-inflammatory hyperpigmentation (PIH) is brown or grey-brown. It is excess melanin left behind after inflammation, and it is more common and more persistent on deeper skin tones.

Post-inflammatory erythema (PIE) is pink or red. It is dilated capillaries rather than pigment, and it is more visible on lighter skin tones.

What works on marks

Both fade on their own — PIH over three to twelve months, PIE often faster. You can speed this up:

  • Sunscreen, daily. Non-negotiable. UV drives pigment production and will keep re-darkening a mark faster than any treatment fades it.
  • Niacinamide. Interrupts pigment transfer and calms the inflammation underneath.
  • Alpha arbutin. Works directly on the pigment pathway.
  • Patience. Twelve weeks minimum before you assess.

What works on scars

Honestly: not skincare. Indented (atrophic) scars are missing collagen; raised ones are excess. Topicals cannot rebuild that structure. Effective options are procedural — microneedling, fractional lasers, subcision, TCA CROSS — and belong with a qualified dermatologist.

Any cream promising to erase scars is describing marks, or describing nothing.

The prevention that actually matters

Both marks and scars start with inflammation, and picking multiplies it. Squeezing a spot pushes inflammatory material deeper, which is the single biggest driver of both outcomes. Treat the acne early, wear sunscreen, and keep your hands down.

If your acne is cystic or scarring, see a dermatologist sooner rather than later. Prescription treatment early prevents damage that no amount of skincare can undo afterwards.
